The TN Employer Purchase – Option Plan Exemption Notice Form IN-1461 is a representation that the issuer intends to offer securities in or into the state in reliance on the exemption pursuant to Tennessee Code § 48-1-103(b)(9)(A)(iii).  In addition to the submission of Form IN-1461 issuers should complete and submit Tennessee Uniform Consent to Service of Process Form U-2.  Members also downloaded Tennessee Uniform Consent to Service of Process Form U-2.
