MS- Mississippi Notice of Cancellation of Investment Form – Exhibit B

The MS Notice of Cancellation of Investment Form – Exhibit B should be completed by an investor that would like to cancel their investment in a crowdfunding offering pursuant to Mississippi Administrative Code, Part 14, Rule 7.21.  The form should be filed with the issuer and the escrow agent and once received the issuer must cancel the investment commitment of the investor and direct the escrow agent to return any funds committed by the investor.
