FL- Florida Issuer-Dealer Compliance, Form OFR-DA-5-91

The FL Issuer-Dealer Compliance, Form OFR-DA-5-91 should be completed by the applicant of the issuer or by a registered dealer identifying the number of registered persons associated with an offering pursuant to Florida Statute, Section 517.12(1).  Portions of the form can be submitted electronically through the Regulatory Enforcement and Licensing (REAL) System, but the signature and notarization should be completed on a printed form.
