FL- Florida Disclosure of Business Activities in Cuba Form

The FL Disclosure of Business Activities in Cuba Form should be completed by an issuer of securities if they commenced business with the government of Cuba or any person or affiliate in Cuba.  The form should be used to report any material changes to previously reported business activities in Cuba.  The form should be submitted to the Florida Office of Financial Regulation within 90 days after commencing business or within 90 days of a change to previously reported information.  Members also downloaded Florida Application for Registration of Securities Form OFR-S-1-91.
