The OK Model Accredited Investor Exemption Uniform Notice of Transaction Form is a representation that the issuer of the sale of securities ensures that the sale will be made available to only accredited investors as defined by the U.S. Code of Federal Regulations 17 CFR 230.501(a) and the Oklahoma Uniform Securities Act.  The Oklahoma Model Accredited Investor Exemption Uniform Notice of Transaction Form must be accompanied by the Oklahoma Accredited Investor Exemption Form and the Uniform Consent to Service of Process Form U-2.  Members also downloaded Oklahoma Uniform Consent to Service of Process Form U-2 or Oklahoma Accredited Investor Exemption Form.
