OK- Oklahoma Accredited Investor Exemption Form

The OK Accredited Investor Exemption Form is a representation that the issuer of the sale of securities ensures that the sale will be made available to only accredited investors pursuant to the Oklahoma Uniform Securities Act.  The Accredited Investor Exemption Form must be accompanied by the Model Accredited Investor Exemption Uniform Notice of Transaction Form and the Uniform Consent to Service of Process Form U-2.  Filings should be made no later than 15 days after the first sale of securities subject to the Oklahoma Uniform Securities Act.  Members also downloaded Oklahoma Uniform Consent to Service of Process Form U-2 or Oklahoma Model Accredited Investor Exemption Uniform Notice of Transaction Form.
