The OR Application to Register Securities Form should be completed and filed by an issuer seeking to register offerings pursuant to Oregon Administrative Rule (OAR) 441.  Offerings registered pursuant to OAR 441 -065-0220 must submit offering documents.  However, offerings registered pursuant to OAR 441-065- 0230 do not require submission of documents unless requested by the Department of Consumer and Business Services.
