The OR Investment Adviser Attestation of Errors and Omissions Insurance Form should be completed and filed by investment adviser applicants to demonstrate compliance with Oregon Revised Statute 59.175(5) and Oregon Administrative Rule 441-175-0185. The form may be used in conjunction with an insurance policy declaration page or certificate of liability. The form should also be used inform the Oregon Division of Financial Regulation of any changes in insurance.
